Type : Used to select the certificate type from the list.(Ex : Declaration,Actual)

Organization Field : This Field is used to select the organization which you are going to process the document.

Tax Scheme : Used to select the particular tax scheme.(Refer Master Window)

Employee : Used to select the particular employee name.(Refer Employee Master)

Amount : Used to mention the tax amount.
